Currently, the SRC operates at two sites in Downtown Los Angeles: 1) the USC + Eisner Family Medicine Center, a Federally Qualified Health Center, and 2) the John Wesley Community Health Center for Community Health (CCH) Downtown, a private nonprofit health clinic located on Skid Row. The SRC utilizes an interdisciplinary team-based approach in a primary care setting while emphasizing patient education and comprehensive patient care. There are multiple clinic teams per site, with each team consisting of 1 faculty preceptor, 1 Care Team coordinator, 2 preclinical medical students, 1 clinical medical student, 1 pharmacy student, 1 occupational therapy student, and 1 physician assistant student.
The population we serve
The clinic serves current USC + Eisner Family Medicine Center and CCH patients including the underserved, homeless, and uninsured. Specifically, we target complex patients with comorbidities that require an in-depth and comprehensive health assessment.
The services the SRC provides to the patients
Services provided to the patients include: a comprehensive history and physical exam, preventative screenings, behavioral counseling, medication reconciliation, wellness education, and lifestyle redesign®.

We are planning to expand to new clinic sites in the Greater Los Angeles area, allowing us to extend our comprehensive healthcare services to additional patient populations. We welcome any institutions interested in collaboration to contact us at our email address.
